Here is a free template for you to start creating your birth preferences! Click the button below to print both sheets. Use the one I filled out as a template and write in the items you want in your birth plan on the blank birth plan.
Although this list isn't all-encompassing, I did my best to give a good example of what a birth plan may look like for someone wanting a very natural and undisturbed birth. This would be a great place to start if you are having a hospital birth, however, there are lots more interventions possible in the hospital that are not included here!
